pretty sure my ocz agility III is on its last legs. Do these numbers confirm it?

not quite sure what they mean but i'm almost certain its fucked.

Any advice would be appreciated.

Yes, backup anything on it asap its probability of failure is close to certain. Those drives were terrible to begin with.
