HTML Form Creator - Teachers fill out form and send into a database

My mom is creating a form for work and istead of sending paper or a word document to the teachers she is sending the form to, she wants a computerized one that can be sent to a database. We cant use access because it has to be accessable by TEACHERS, so I thought, why not HTML. I don't feel like teaching her HTML so I want a program that will create a form/survey type thing (like entering shipping info on amazon). If you know of any that would help.

Also if you know of some other way of writing a form (not HTML) and having it send to a database (dreamweaver?) please add that.

If you are looking for some sort of survey tool, I would sugest Google docs or survey monkey, though I haven't used it my self, I have filled out many of those surveys.
