I have an old HIS 6850 in my rig, whilst gaming on CS:GO and GTA V after a certain amount of time the resolution glitches out as seen in the image I attached to this. Sorry for the quality it was the best I could get.
I've completely factory reset my PC (not because of this but it was happening before that anyway), reinstalled my drivers and I've monitored my temps whilst in these games and it's averaging 60 degrees.
I have no idea what's causing this to happen, I assume it to be the GPU's fault but I have no idea.

Does it only happen when hitting 60 c? From the image looks like a monitor issue. Try another monitor if you have one. Different cable, factory reset the monitor.
Generally GPU that has issues results in distortion is generally worse.
I agree... This does seem monitor related however I wouldn't rule the GPU out it is very old. Wouldn't hurt to go hook your pc up to the TV for a while or something.
You could also burn a CD with Hiren's Boot Disk and try the GPU testing tools on that. Furmark would be good to use as well. See if the GPU does tank after it gets warm or if the monitor tanks after it's been on awhile.
I'm not sure if it's only then, I don't think so however as soon as I booted into CS today it did it. I'll grab another monitor, test that now and say what happens in a couple of hours or so. Thanks.
The Problem you are showing can be caused by a garbled or a unsupported frame rate information being sent to your monitor. for instance if your monitor only takes 59.97 fps in but is being given a true 60 fps this can happen.
Man I hate to tell you but your GPU is about to die, at least from my experience. My 6870 reference did exactly the same thing before kicking the bucket a few minutes later. It still displays video out but, as soon as I try to install driver for it, it just crashes over and over again. I know your GPU is still working but I don't know how much longer will it last.
