Is my OC unstable?

I recently OC'd my FX-6300 to 4.2GHz  I ran prime95 for about 3 hours without any cores failing. The only problem I've had was one lock up on Crysis 3 maxed out. After playing for about 5 hours straight, but haven't had one hiccup since. My question is in Asus Probe II I noticed this today...

[09/19/2013 at 01:32 pm] +12V 7.104 Abnormal
[09/19/2013 at 01:32 pm] +12V 12.046 Normal
[09/19/2013 at 01:54 pm] +12V 0.360 Abnormal
[09/19/2013 at 01:55 pm] +12V 12.046 Normal

Is this bad?

Thanks in advance.

Asus softwhere is useless use hwifo64 keep it running in the background and see what it monitors on your voltage.

I have the same CPU and I reach 4.6 GHz super stable.

Could you also list your MOBO, Cooler, and PSU?

And temps. Use HWMonitor or a similar program.

Sorry people I had a long week, and weekend, but anyways... I'll download hwinfo thanks cooperman. Sapere my whole computer stats is in my profile just click my name. I constantly run HWMonitor my CPU sits at 27c at idle, running games like Crysis for hours it usually never gets hotter then 40c. I also probably should have mention this is my first OC. I don't remember all the settings after I finished the OC if you need those I can get them, cause I may have fudged that up too...

Thanks again.

run memtest+ at boot for 2 complete passes

run Prime 95 small FFTs (the middle option) for 12 hours

aida 64 is the most accurate system monitor

keep in mind the silicon lottery.

sold parts are guaranteed to run stable @ spec on box when they're within temperature parameters. If he has a bad piece of silicon it may not get past that 4.2

What is your voltage? Did you increase it? 1.55 volts is the max safe voltage. Try increasing the voltage and stress testing.

Also try increasing the FSB and reducing the multipliers on HTT and CPUNB and the RAM divider accordingly.

Like this for example:
