This is correct.
2 out of 3 engines failed to re-ignite apparently this was because they didn’t have enough TEA-TEB left to ignite them again.

It carries limited amounts of the stuff, so you can only have a limited number of restarts.

As a result the rocket hit the water going 500km/h a distance away from the barge showering everything with water and debris.

But the engines themselves need to be more durable for re-entry, as the center stage needs to re-enter the atmosphere slightly. If it ran out of some kind of fuel, well, the KSP video I posted around Post 41 was pretty much exactly what happened.
