Custome Windows Themes (Need Help)

Does someone know how to create themes for windows 7 like this (just very simple themes) I always just find the themes for download and no tutorials.

as for themes, I use placebo.

If i want to design skins (create themes myself) do i need an other programm or can i do it with rainmeter or rocketdock (doesnt look like i could)
