Looking for best GPU, my budget is around $230 I preferably want to run Fallout 4 on max and still be good for future proof at least 2 years into it.

if you can pick up a R9-380 or R9-380X. Those are excellent 1080p cards. And they are around $220 to $250.
Pretty much go with what kat said get a 4gb r9 380. Either that or try and risk a used 290.

yeah you cant beat the 380X for the price its also a decent card at 1440P you will have no trouble running most games at high to ultra with it
You can run most games from high to ultra with almost any GPU, be it discrete or integrated. What FPS you'll get and whether it is even nearly playable at whatever resolution you're playing at (I'll assume 1080p) is a whole other story.
To determine how any GPU will work for you, you need to counter in the detail setting you want to use (like you did), but also what resolution you will be playing at and what FPS do you deem playable. As we are talking about a First person shooter here you really need to have atleast a steady 40 and I can guarantee you wont get steady 40FPS with Fallout 4 on Max settings or anywhere close to it with 200$ GPU even if you would buy used and got a really lucky deal.
Also asking for 2 years worth of future proofing is a bit overreaching. I'd be fairly confident you could get that with a Fury X or a 980 Ti, but not much else. You need to settle with the requirements or increase your budget.
Personally I always recommend buying used with patience during the investigation, especially when on a budget.
From what I know (I could absolutely be wrong) we're in an odd transition point where the GTX 7xx series and the R7/R9 2xx series are still widely available, but the new cards (GTX 9xx series and R7/R9 3xx) are being phased in to replace the last generation I believe. There are some sweet deals on "last-gen" cards right now, as I saw a PowerColor 270 on sale for $110 (until it got bumped to $130, still a good deal) which is a 1080p/1440p powerhouse. Also some of the current generation cards are going "on sale" now but be careful.
If you can snatch one of those PowerColor 270s on sale ($110), a 280X ($180~), or another gaming-level last generation card on sale or clearance it should be more than enough.
This happened a year ago. We're now almost to the point that the GTX 900 and R9 300 series are over the hump and will be phased out in less than a year. Last gen cards are still widely available, but for the most part GTX 700 series cards are not worth their resale value because nVidia has done their very best to destroy performance to make the GTX 900 series more appealing. On the reverse side, older AMD cards dropped in price much faster, and continue to receive incremental driver updates to improve performance in newer titles.
You can get an R9 290 on eBay for $250, and it will slaughter the GTX 960 and R9 380X, which both occupy that price point. With that said, you can get the GTX 960 or R9 380 (non-X) for $200 as well depending on the vendor, so if you don't quite have $250 to drop on a used card there are still new and respectable* options available.
*Respectable is in remark to the R9 380. The GTX 960 was a disappointment the moment they released it...

I'm rocking a 250X and doing fine. a 360 or 370 are under $175 USD and really good for what they are.
Here's an XFX R9 290 from a decent enough seller for $260.
And here's a reference R9 290X for $260. Obviously the cooler is an issue, but you can upgrade that later. If you decrease clockrates and operating voltage to that of the R9 290, you can actually keep the card from hitting that legendary 95C thermal throttle limit. The performance will still be great for that kind of money.
I'm also looking for a budget video card but my budget is much lower: can't go over $120. Anyone have suggestions.
R9 270 / Radeon HD 7870. Or save another $20 and get a Radeon HD 7950.
You can also sometimes find the GTX 670 and GTX 680 at that price, but it's not often.
I got a gtx 770 for 135. It was refurbished and open box. They literally handed me a box with the card and not even an anti static bag. I was actually looking at getting a gt 740 or gtx 750 for an htpc build that would play csgo. I took a chance but I was a little concerned when I first booted it up. So far it works but I don't plan on overclocking it....ever:)
It was at Microcenter and had to drive 2 hrs each way to Jersey. I live in Pa. I think they still have the non-open box ones for 169.
I was gonna go with an open box 740 for 70 bucks of a refurb reference GT 560ti for 59 bucks till I saw it.
Best Buy had on clearance either the 750 or 950 with free shipping for 119.
Took me 2 months or more of loking but just keep looking and You can score something killer for 120.
I'm pretty sure I have a 750 (non Ti) for $120 in my current build, I intend on buying the day after New Year's so don't buy 'em and make me pick another card xD

Closest one is 2 hours away, talked to my mom and she said she isn't driving me up there. And I'm not old enough to drive.
I can understand . 120 is hard cause you already have the 750. I have no idea if you can run twin 750's or even how they would perform.

I'm actually a few bucks over budget but I got my mom to cover the extra change. I just couldn't afford to go the extra $30 to get a 950. In fact I had to cut corners on just about everything while still maintaining quality parts.
