4690k overheating with stock cooler?

I recently noticed that my Intel Core i5 4690k (which is NOT overclocked) has been overheating recently. As I type this, HWMonitor is saying that my CPU is anywhere from 40 C to 50C. And that is about the same when my system is completely at idle, maybe going down into the high 30's. Also, when I launch Far Cry 3 and play for about 5 minutes, the temps jump all the way up 90C to 95C. I am using the Intel stock cooler (though I am planning to upgrade to a decent air cooler for Christmas, like the Dark Rock 3), and I checked to see if the stock cooler was loose, but it didn't budge and is rock solid in the motherboard. So does anyone know what might be wrong?

I know you said it is not overclocked, but you should get in the bios and make sure everything is set to default/auto. Probably wouldn't hurt to go ahead and remove the cooler and clean off old paste apply new and reseat.

For refrence i have a 4770k OC'd to 4.5 @ 1.28v with an H100i and it idles arounf 35-40c and never goes hotter than about 65-67c.

Also use different temp monitors to see if they are reporting different values. Some software uses the motherboard sensor, some use the one inside the cpu itself.
I recommend CoreTemp or Open Hardware Monitor. The later being one of my favorite monitoring programs around. Also you should use CPU-Z to check your voltage and values the CPU is reporting.
Also check in the bios hardware monitor and see what it says the temps are.
Is your M/B itself getting as hot as the CPU or anywhere near?

Last thing you can do is borrow or purchase a laser temp gun -  http://www.amazon.com/HDE-Non-Contact-Temperature-Thermometer-Targeting/dp/B002YE3FS4 $20 on amazon.

I actually went into the bios last night and made sure everything was on auto/stock. Also, HWMonitor, RealTemp, and Asus's AI Suite are all giving me the same temps as described above. And CPU-Z says that at idle, the voltage is at around .8. And HWMonitor also says that my motherboard's temp. is around 25C (which I think is normal). So you think the best option is to reapply the cooler with new thermal paste?

Yeah that would be my next step.
How old is the processor?

I have had issues with the stock cooler before where due to heat the cooler kinda "glued" to the processor and seemed mounted solid, but in reality those stupid plastic clips intel uses didn't click through the holes right and it wasn't actually seated fully.

I had this problem ,

Take off cooler , and reapply thermal paste , make sure you push properly and there is pressure between the cooler and CPU.

You should see 32-73°c idle-load .

^^^^this^^^^  and yes ... upgrade to the dark pro or Noctua

I have the same processor as you, i5 4690k. I ditched the stock cooler for a corsair h100i with noctua fans. I idle at 22 degrees Celsius, while OC'ed to 4.5 GHz, and under most gaming loads i dont exceed 60 degrees Celsius. Only in synthetics do i hit around ~75. If you can get the same cooler, go for it, if not the dark pro will be awesome.

I have the same issue my friend, but I have an i7-4790k and when i went into bf4 temps hit 88c-94c. I took the stock cooler off and reapplied some thermal paste and still same thing, so I sent it back for a replacement. Still in the process of waiting :(

Buy a decent air cooler like  ... Noctua or Dark Rock Pro or Silver Arrow
